No middle initial was used. The first thing the title company asked is what is Jack’s middle initial.

The next question was which Jack Smith is he that lives at the house address. It seems there were 3.
- Jack Smith
- Jack Smith Jr
- Jack Smith III
And to make matters worse, Jack Smith was deceased. We needed a death certificate.
